Overview of Assessment

Twin Cities Lyme Foundation’s objective is to provide access to proper diagnosis for individuals that believe they are experiencing symptoms of Lyme disease. We do this through our participating Lyme Literate Practitioner (LLP).

Our assessment is not meant to diagnose Lyme disease, rather understand whether the symptoms one is experiencing may relate to Lyme disease and warrant consultation/examination.

The assessment will present a series of general health related questions. The cumulative answers to the questions will determine whether or not a consultation/examination is a reasonable next step to determining whether or not an individual may carry the bacterial species related to Lyme disease.

Should the results of the on-line evaluation indicate that a consultation/examination is warranted, you will receive an e-mail as soon as possible with instructions on further steps to be connected to an LLP who can provide you with and educated Lyme literate diagnosis.

Symptoms Assessment

The following is a listing of symptoms that an individual with Lyme disease may experience. Please complete the following assessment by rating the severity of each of the following symptoms as they may or may not apply to you.

Heightened allergic sensitivities

Jaw pain/stiffness (“TMJ”)

Change in smell or taste

Green mucus in eyes

Diminished exercise tolerance

Malaise

Aggressive behavior/impulsiveness

Suicidal thoughts (rare cases of suicide)

Overemotional reactions, crying easily

Feeling as though you are losing your mind

Compulsive outbursts of profanity

Obsessive-compulsive behavior

Unexplained menstrual pain, irregularity (female)

Reproduction problems, such as miscarriage, stillbirth, premature birth, neonatal (female)

Change of hair color

Extreme PMS symptoms (female)

Pelvic pain (female)

Testicular or pelvic pain (male)

Sound sensitivity

Shifting joint pains

Poor muscle coordination, loss of reflexes

Burning sensations in feet

Holes in tongue

Weakness or paralysis of limbs

Seizures, stroke

Encephalitis (inflammation of the brain)

Meningitis (inflammation of the protective membrane around the brain)

Encephalomyelitis (inflammation of the brain and spinal cord)

Cysts in underarms

Difficulty with multitasking

Difficulty with organization and planning

Slowed speed of processing

Erythema Migrans (rash)

Flu-like symptoms (early in the illness)

Sleepwalking

Symptoms seem to change or come and go

Decreased interest in play (children)

Dysfunction of the thyroid (under or over active)

Bladder & kidney problems (including bed wetting, urgency/frequency to urinate)

Newly developed beef or meat allergy

Blackened toenails

Persistant swollen glands

Sore throat

Hearing: buzzing, ringing, decreased hearing

Increased motion sickness, vertigo, spinning

Off balance "tippy feeling"

Light headedness, wooziness, unavoidable need to sit or lie

Tingling, numbness, burning or stabbing sensations, shooting pains, skin hypersensitivity

Facial paralysis, Bells Palsey

Dental pain

Neck creaks and cracks, neck pain

Fatigue, tired, poor stamina

Insomnia, fractionated sleep, early awakening

Excessive night time sleep

Napping during the day

Unexplained weight gain

Unexplained weight loss

Unexplained hair loss

Pain in genital area

Unexplained milk production, breast pain

Irritable bladder or bladder dysfunction

Erectile dysfunction

Loss of libido

Queasy stomach or nausea

Heartburn, stomach pain

Constipation

Diarrhea

Low abdominal pain, cramps

Heart murmur or valve prolapse

Heart palpitaions or skips

"Heart Block" on EKG

Chest wall pain or ribs sore

Head congestion

Breathlessness, "air hunger", unexplained chronic cough

Night sweats

Exaggerated symptoms or worse hangover from alcohol

Symptom flares every 4 weeks
